We suggest everyone call the back of their insurance card and request their behavioral health benefits. This way, there are no surprises with co-pays and deductibles, or the possibility of those services not being included in your plan.
Cash Rate for Dr. Bonner: $130

The cash rate will be charged if your insurance has lapsed, or it is not a covered service even if the original insurance inquiry gave a different estimate for each session rendered.
Sessions will vary from weekly, biweekly, monthly, or as needed based on your individual goals. If you have insurance but choose not to use it, the rate will be the same as the cash rate.
Testing Fees:
Bariatric Assessments: $250 This service is not covered by insurance. This includes the assessment and report for your provider.
ADHD testing: This is not covered by some insurances. Testing services are $150 an hour which is for each assessment, testing, results and report writing sessions.
In a rush? Need the testing and results quickly? Most testing and results can take about 4-6weeks for completion based on the complexity of the case. For quicker results, there is $300 rush fee.
Court Fees: $2000 retainer in advance. An hourly rate of $250 will be charged for all travel, court preparation, correspondence, court appearances, and court testimony. Insurance companies will not cover these costs, and they will be billed to the client or guardian.
No Show/Late Cancellation Fees: Please cancel sessions 24 hours in advance. Failure to do so will result in the cash pay rate ($115-$130) being charged. Since insurance companies do not cover missed appointments, you are responsible for the full payment. After two missed appointments or late cancels, further services can be canceled until treatment becomes a priority.
A credit card will be kept on file through the Electronic Health Records for ease of payments. You can submit an alternative form of payment, or the card on file will be charged at the end of the session.
